Hospital and Housing Supports Services Integration Pilot participants, including participating healthcare settings and NJ FamilyCare Housing Supports Program providers serving NJ FamilyCare members with complex housing needs.
Grant funds may support reasonable and necessary costs directly related to providing technical assistance, supporting pilot implementation and shared learning, and conducting the mixed-methods assessment. Grant funds may support eligible costs across four categories: 1) Personnel Costs; 2) Operating Costs; 3) Purchased Services; and 4) Administrative Costs, provided expenditures directly support the goals and objectives of the Hospital and Housing Supports Services Integration Pilot. Applicants must have at least three years of experience providing technical assistance, implementation support, or systems-level capacity building in healthcare, housing/homelessness, social services, or cross-sector settings. Applicants must also demonstrate existing or prior relationships with New Jersey healthcare and Housing Supports providers (or comparable organizations) and commit to serving as the technical assistance provider through the pilot period, with the possibility of extension.
To fund a qualified organization to provide technical assistance to hospital–Housing Supports provider partnerships participating in the Hospital and Housing Supports Services Integration Pilot. The selected organization will support pilot implementation, facilitate shared learning, and conduct a mixed-methods assessment to identify effective practices, challenges, and lessons that can inform future program design and potential statewide scaling. 11/1/2026-6/30/2028

NJ FamilyCare members with complex housing needs who are experiencing homelessness, housing instability, or other housing-related needs and who interact with participating hospital or healthcare settings. The pilot is intended to connect eligible members to timely, coordinated Housing Supports Program services and improve housing and health outcomes.
Grant funds may support technology; development of business or operational practices; workforce development; and outreach, education, and stakeholder convening necessary to develop and implement effective integration between participating healthcare settings and NJ FamilyCare Housing Supports Program services. Eligible applicants must be nonprofit NJ FamilyCare Housing Supports Program providers applying in partnership with an eligible hospital or other healthcare setting. Applicants must demonstrate an established partnership and the capacity and commitment to implement the proposed hospital–housing integration model.
To fund partnerships between hospitals/healthcare settings and NJ FamilyCare Housing Supports providers to better identify, engage, and connect eligible members to ongoing housing support services. See RFP. CBOs and other eligible entities providing homelessness and housing support services aligned with the Housing Supports program of NJFamilyCare. Non-profit organizations that are either approved by NJ FamilyCare or eligible for approval to provide pre-tenancy and tenancy sustaining services.
The NJ Division of Medical Assistance and Health Services (DMAHS) has launched a new Housing Supports program to help NJ FamilyCare members find and maintain housing. To increase member access to quality services, DMAHS has partnered with DCA to invest in providers who are interested in joining or expanding their role in the program Under the NJ FamilyCare Housing Supports Provider Capacity Expansion Grants Program, DCA and DMAHS seek to partner with two tracks of providers:
“Start-up” Track: Prospective & Early Stage Housing Supports Providers (Not yet having credentialed & contracted as a Housing Supports provider with at least 1 MCO). These are potential NJ FamilyCare Housing Supports providers (e.g., CBOs, shelters, supportive housing organizations, case management organizations) who are still in foundational stages of becoming a NJ FamilyCare Housing Supports provider and have not yet credentialed & contracted with at least 1 MCO to deliver housing services). Grants are intended to: Enable Housing Supports program readiness and offset startup costs (e.g., Medicaid enrollment, MCO contracting, development of new IT system capabilities, participation in required trainings). Provide seed capital to support the hiring of new staff—both administrative and frontline service providers— to support the commencement of service delivery to NJ FamilyCare members. “Expansion” Track: MCO-contracted Housing Supports Providers (providers credentialed & contracted with 1 or more MCOs for housing services): These are providers who are already enrolled through Gainwell in the Housing Supports program and are in-network with at least one MCO. Grants are intended to: Incentivize investment in the capabilities, infrastructure, and staffing needed to expand their capacity to serve NJ FamilyCare members and improve the value of those services to NJ FamilyCare members.
